Temple 17 of the Chichibu Kannon pilgrimage, a city-designated historic site whose bronze bell is a prefectural cultural property. Chichibu City lists it on its Anohana course. It is an active temple.

What the city's heritage listing says
Chichibu City's board of education lists the temple, at 21-3 Sakuragi-cho, as a city-designated historic site, designated on January 25, 1965. The Kannon hall has a square roof and a freestanding four-bay-by-four-bay plan, and it stands on an earthen platform about 1 meter high. The inner sanctuary is enclosed by doors at the front and sides, and, in the style of an old Amida hall, has a corridor for walking and chanting.
The main image is a standing eleven-faced Kannon, 55 centimeters tall, made of joined wood. Another seated eleven-faced Kannon is inscribed in ink with an eye-opening date of March 23, 1593. The bronze bell is a prefectural tangible cultural property.
Reaching the temple by bus
The city's model course for Anohana and Kokosake (another anime set in Chichibu) uses a bus stop called Kasho 17-ban Iriguchi, meaning the entrance to Temple 17. It is about a 4-minute ride on the Seibu Kanko Bus Ogano line from Chichibu Bridge toward Seibu-Chichibu Station. The course allows roughly 90 minutes for visiting the temple and walking on to Chichibu Station and the local products center; bus times can change, so check them before you go.
